By completing this form you agree to provide the Forum organisers timely information about all changes.
In case of the cancellation of your participation in the Forum you agree to offer organisers for consideration an alternative speaker (subject to his/her availability and ability to travel).

ACCEPTANCE AND REGISTRATION
Once your application has been approved, you will be requested to register and pay online (unless agreed otherwise) for your participation in the Forum. Electronic invoice will be generated and sent to email which you will provide in the registration form. Please let us know if you require invoice by post.
